REQUIRED MEDICAL FORMS
The Department of Intercollegiate Athletics wishes to welcome you as a sports camp participant. Every camp carries with it some degree of risk to the participant because of the physical nature of the activities. The UT Sports Medicine Staff in cooperation with the University Health Services and a network of consulting physicians and certified athletic trainers provides specialized services, care, and supervision to safeguard your health and well being.
MEDICAL CARE / INSURANCE
Participant’s parent or legal guardian must complete the required medical forms. Note: A physician’s signature is required on one of these forms; however, the participant’s current school year
physical may be attached in lieu of a doctor’s signature if the completion date
for physical is within 14 months prior to the camp end date. The medical forms must be submitted prior to registration. Each participant must be covered by his legal guardian’s insurance before participating. Medical aid is provided on site by a licensed athletics trainer or the Texas Sports & Family Medicine or a local hospital. Campers’ parents/guardians are billed directly for all medical services rendered by Texas Sports & Family Medicine or a local hospital, and not covered by their insurance.
To facilitate this care, we are requesting that you complete a number of medical forms. Campers will not be permitted to participate in any activities until all forms are completed and are on file with the camp supervisors. The following forms may be downloaded for your review and completion:
